A Case-report of Concurrent Pulmonary and Cerebral Lesions in a Patient with Polymyositis: Invasive Aspergillosis or Astrocytoma?

Introduction: Polymyositis is an idiopathic inflammatory myopathy that mainly manifests itself with muscle weakness. Patients polymyositis have a higher risk of developing infections and malignancies. We report concurrent pulmonary cerebral lesions in patient many diagnostic challenges. Case Presentation: A 56-year-old woman complained productive cough dyspnea from two weeks ago. Her symptoms gradually progressed until sudden loss consciousness occurred. She was known case treated oral prednisolone. Imaging revealed lesions. Initially, the underwent empirical therapy. However, bronchoscopy because she did not respond to treatment. Specimens obtained respiratory secretions branched septate hyphae, culture positive for Aspergillus fumigatus. diagnosed invasive aspergillosis, so we replaced therapy voriconazole. After three months, lung improved, but number extent increased. Finally, after stereotactic biopsy, astrocytoma became candidate radiotherapy.
